By submitting this form, I confirm that the information I have provided is true and accurate to the best of my knowledge and that I am authorised to provide the photographs and information submitted.

I give Watalot Group, WataSport and their authorised production, marketing, distribution and retail partners permission to use my submitted name, player information and approved photographs for the design, manufacture, promotion, marketing, distribution and sale of WataSport products, including but not limited to trading cards, stickers, magazines, posters, games and related collectible products.

I understand that I am providing this permission to Watalot Group without compensation being due to me from Watalot Group, unless Watalot Group and I have entered into a separate written agreement stating otherwise.

I understand that any payment, royalty, fee or other arrangement I may have separately agreed with my club, team, association, players' organisation, agent, representative or another third party is separate from Watalot Group and does not create any payment obligation or agreement with Watalot Group.

I understand that submitting this form does not guarantee that I will be selected, featured or represented on a Watalot or WataSport product. Watalot Group may determine which submitted players, information and approved photographs are used and the products in which they appear.

If I am selected, I understand that Watalot Group cannot guarantee or provide an exact number of cards, stickers or other products that will be printed, manufactured, distributed or sold featuring me. Production quantities may vary according to the product, retailer, distribution requirements and demand.

I understand that submitting this form does not automatically entitle me to free copies of any product. However, if I am featured and request a copy of my individual card or sticker, Watalot Group will endeavour, where reasonably possible and subject to availability, to provide me with a copy.
